Help with student research

Hello, im conducting research for my course to hopefully be a mental health nurse, so if you could help me with these short questionnaires i would be very greatful! please copy and paste questions and fill them out. either reply on here or as a privet message to me if you like. thank you!


1) Do you take part in regular meditation (eg more than 3 times a week)?

2) Do you take part in regular exercise (eg more than 3 times a week)?

3) In most cases, do you eat 5 portions of fruit and vegetables per day?

4) Do you work full time hours? (Eg over 37.5 hours a week)? If yes do
you also regularly do overtime?
5) On average, would you say you ‘rush’ through the week?

6) Do you take time out of the week to do things that you enjoy and that make you feel rejuvenated?

7) At work, are you active or do you spend most of the day inactive (eg at a desk)?

8) Do you spend your leisure time active or lounging?

9) Do you Smoke?

10) Do you drink most weekends (more than 6 units in a session)?

11) Do you, more often than not, get at least 8 hours sleep per night?

12) Do you eat quick fix meals on a regular basis (eg microwave meals and fast foods)?

13) Please state your height and weight. …….Lbs and …Ft…In


Also could you please rate these questions from 0 (being none) and 3 being severe on a day by day basis eg some day would be 2, every day a 3, and now and then a 1. thank you.

1) little or no interest in doing things
2)feeling down, depressed and hopeless
3)trouble falling or staying asleep or sleeping to much
4) feeling tired having little energy
5) feeling bad about yourself
6) poor appitite or over eating
7) touble concentrating
8) slowed speach and/or movement or oppisite, very fidgety.
9) thoughts of scuicide.
10) do these problems effect you day to day functioning.
