Experiences Getting Diagnosed with SA

I've never been diagnosed with SA but I'm sure I suffer from it and now, at the age of 30, I would like to see a professional. This anxiety has been going on for far too long. Of course, now I suffer anxiety from actually visiting a professional (I've never had counseling before in my life) and need to know what to do.

So, I want to know, since it seems like most of you are very well-educated about your SA:

1) What type of psychologist should I see? There are so many titles (MSW, etc). I'm doing cold calls here and I'm embarrased to ask anyone at work, in family, etc for referrals. I'm basically looking up names in the yellow pages.

2). Is medication a good idea? What are the typical medications for SA?

3). How is one tested? Did you fill out a questionairre? Did you just describe your anxiety and a mental health professional said "ah, yes, you suffer from Social Anxiety!" What are your counseling sessions like?

I am in the same boat, I have not been diagnosed and know for a fact, by research that I have SA ( I'm %99.9 sure). I too need help, I have been to a psychiatrist and a therapist but they gave up on me because I did not get better, now I am worst.
